To Contact our Nurse On-Call
To provide you with the greatest support possible, we offer an on-call system for assistance after business hours. You will receive a telephone sticker bearing the particular office number that you should use.
When using our on-call service, please follow these steps:
- Briefly describe the problem.
- Ask the operator to contact the appropriate home health nurse on-call.
- Give the operator your name and telephone number for the return call.
- Notify the operator if you are receiving services from one of our specialty teams – i.e., special care or private services.
- If you have not heard from the on-call nurse within 30 minutes,
please call again.
In case of emergency
Unfortunately, our home health office cannot provide emergency care to handle such symptoms as chest pains, severe shortness of breath, bleeding and related problems. We urge you to immediately call an ambulance service or the fire or police departments in such situations.
Reporting A Problem Or Concern
At Fayette Home Care and Hospice, we recognize your right to voice any problems or concerns. To do so, please contact the Director of your service area.
If you are not satisfied with the results you have received, please contact the Executive Director at (724) 439-1610. All complaints received by the agency will be documented and investigated. Follow-up will be provided with your input until the problem is resolved.
If you feel your problem needs further investigation, please contact the Pennsylvania Department of Health's Home Health Hotline at 1-800-222-0989 between the hours of 8:30 a.m. and 4:30 p.m., Monday through Friday. After business hours and on weekends and holidays, all hotline calls are placed on
an answering machine and reviewed
the morning of the next working day.
Fayette Home Care and Hospice is accredited by the Joint Commission on Accreditation of Healthcare Organizations. You may also report a concern or register complaints to JCAHO by either calling 1-800-994-6610 or e-mailing
Make a Referral
Anybody can make a referral to Fayette Home Care and Hospice. To make a referral, dial (724) 439-1610 or (888) 402-2233. Any member of our intake staff would be happy to assist you and answer any questions regarding insurance.
