A Team Approach To Your Care


At Fayette Home Care and Hospice, we've assembled a skilled team of licensed and
certified nurses, therapists, social workers, dietitians, technicians, aides and volunteers to assure you the highest level of in-home care. Their services are provided in conjunction with a care plan, which has been developed under your physician's direction, and which enlists you and your family's help to achieve its goals. Through
this team approach we hope to reduce the length
of a hospital stay, prevent further hospitalization
and facilitate the speed and completeness
of recuperation.

Our Goal: Your Independence


Your physician is regularly apprised of your progress. As your condition improves, our services are reduced. Our most important goal is to facilitate your recovery and return to independence as efficiently as possible. To achieve this goal, our staff of registered nurses offers a variety of in-home, skilled services, including:
  • Recuperative care for people who have suffered a stroke or heart attack
  • Wound and dressing care
  • Administration of injections
  • Catheter care
  • Patient and caregiver instruction
  • Vital signs monitoring
  • Intravenous therapy
  • Diabetic care
  • Maternal/Child care
  • Palliative care
  • Psychiatric nursing care
Our nurses are also trained to administer IV antibiotic and chemotherapy treatments, and to manage tube feedings. We also provide nursing services
for patients with colostomies and related conditions, and to handle special
skin problems.

Under your physician's direction, a registered nurse can provide these services:
  • Instructions to patient, family or caregiver regarding in-home nursing needs
  • Administration of intravenous medication
  • Drawing blood for lab work
  • Reporting to the patient's physician regarding status and progress
  • Coordinating with rehabilitation team members providing various therapies
  • Managing catheter changes and wound care

Enterostomal Therapy


Our ostomy nurse provides specialized treatment, education and counseling services that include:
  • Evaluating stomas to ensure appropriate appliances
  • Planning and executing treatment programs
  • Training patients and caregivers in ostomy care
  • Assessing and initiating wound care, pressure sore management and self-catheterization
  • Providing counseling services for patients with radical abdominal or bladder surgeries

Infection Control And Safety


Your health and safety are the foremost concerns of our staff. To ensure your protection, as well as our staff's, your home health caregiver may wear gloves and other safety apparel during treatment. You will also be given specific instructions by our staff regarding measures you can take to prevent infection, such as hand washing, proper disposal of dressings, and other measures pertinent to
your safety.

Our staff can also assist you and your family members in recognizing potential hazards in the home, and provide recommendations for resolving them and avoiding possible injury. Our nurse will evaluate your home environment and provide safety precautions.
